Special Needs LPN

This position serves as both a childcare staff member and provides a supportive role in the delivery of physical and mental health treatment to the youth in an RTF Unit setting. This position is designed to provide the daily services of safety, security and structure to youth, as well as, medication administration/administration oversight of staff, persons served mental and physical health education, parent education and treatment coordination, discharge planning, referral support and patient advocacy

- As needed, attend on and off campus medical appointments with persons served.

- Monitor vitals and coordinate phlebotomy services based on best practice

- Oversee the diabetic care

- Complete assessments such as AIMS, Beck Depression Inventory and other mental health screening/assessment tools.

- Educate youth on topics related to diagnosis, physical/sexual health, wellness, hygiene, medications/adherence/side effects, and/or relevant topics.

- Promote staff education on use of medications, medication distribution, safety, documentation and side effects

- Teach and model appropriate social behaviors.

Minimum of an LPN license in the Commonwealth of PA

PA driver's license

Minimum of one year of LPN experience in the field and experience working with individuals with mental health diagnosis and intellectual disabilities.
